This pave the way to multiple subtitles: each registered spu channel has now
its own clock.
And fixes OSD subpictures date being messed up by the decoder clock.

Every registered channels need to be unregistered. The first 3 OSD channels
don't need to be unregistered since they are automatically registered.
sput_t now use a vector of struct spu_channel to keep its channels. This struct
will contain parameters specific to a channel like the clock/delay/rate.
The channel_id is now a ssize_t, -1 being invalid, 0, 1, 2 being the OSD
channels, and [3; SSIZE_MAX] being registered spu channels.
ES_OUT_VOUT_FLUSH_OVERLAY is renamed to ES_OUT_VOUT_DEL_OVERLAY since this
control is only used to delete an overlay.

It is possible, that some minor piece of code was badly tracked, for
some reasons (SVN, mainly) or that some small developers did not answer.
However, as an "œuvre collective", defined as in "CPI 113-2 alinéa 3",
and seeing "Cour. Cass. 17 Mai 1978", and seeing that the editor and
the very vast majority of developers have agreed (> 99.99% of the code,
> 99% of developers), we are fine here.
I have merged spu_RenderSubpictures and spu_SortSubpictures and removed the
blending from spu_RenderSubpictures. No functionnal changes except maybe
a bit for the transcode plugin.
* Headers in include must contain the structures and prototypes needed
by the plugins and should be named vlc_*.
* Headers in include/vlc must contain the structures and prototypes
needed by external libvlc clients
* Moved and renamed some things in headers.
- Removed vlc_cpu.h, vlc_error.h (merged in vlc_common)
- Removed snapshot.h (merged in vlc_vout.h)
- Removed vlc_spu.h (merged in vlc_osd.h)
- Removed intf_eject.h and vlc_interaction.h (merged in vlc_interface)
- Moved all internal headers to src

This commit has a 99% probability of breaking the build, 0.1% of killing
your cat and 0% of getting you hot chicks.
reuses a given picture_t without allocating a new one.
* modules/video_output/picture.c: New --picture-height and width options
to do the scaling in the vout thread. Misc bug corrections.
* modules/video_filter/mosaic.c: Less overhead.
* modules/codec/ffmpeg/video_filter.c: Do the scaling before the conversion
in case the original size is bigger than the destination.
