ee964145b5 lavc: remove unused traces of fmtconvert usage 
Those decoders have been switched to float output and so do not use
fmtconvert anymore.

f20518568a build: Split WMA frequencies into a separate object file 
These are the only WMA bits shared with binkaudio. Splitting them off
reduces the binnkaudio dependency on general WMA code.

5959bfaca3 floatdsp: move butterflies_float from dsputil to avfloatdsp. 
This makes wmadec/enc, twinvq and mpegaudiodec (i.e. mp2/mp3)
independent of dsputil.

c73d99e672 Separate format conversion DSP functions from DSPContext.

987903826b Globally rename the header inclusion guard names. 
Consistently apply this rule: the guard name is obtained from the
filename by stripping the leading "lib", converting '/' and '.'  to
'_' and uppercasing the resulting name. Guard names in the root
directory have to be prefixed by "FFMPEG_".
