Andreas Rheinhardt 790f793844 avutil/common: Don't auto-include mem.h
There are lots of files that don't need it: The number of object
files that actually need it went down from 2011 to 884 here.

Keep it for external users in order to not cause breakages.

Also improve the other headers a bit while just at it.

Andreas Rheinhardt 4a4dcde339 avformat/internal: Move FF_FMT_INIT_CLEANUP to demux.h
and rename it to FF_INFMT_INIT_CLEANUP. This flag is demuxer-only,
so this is the more appropriate place for it.
This does not preclude adding internal flags common to both
demuxer and muxer in the future.

Andreas Rheinhardt b800327f4c avformat/avformat: Add FFInputFormat, hide internals of AVInputFormat
This commit does for AVInputFormat what commit
59c9dc82f4 did for AVOutputFormat:
It adds a new type FFInputFormat, moves all the internals
of AVInputFormat to it and adds a now reduced AVInputFormat
as first member.

This does not affect/improve extensibility of both public
or private fields for demuxers (it is still a mess due to lavd).

This is possible since 50f34172e0
(which removed the last usage of an internal field of AVInputFormat
in fftools).

(Hint: tools/probetest.c accesses the internals of FFInputFormat
as well, but given that it is a testing tool this is not considered
a problem.)

Andreas Rheinhardt 843fe314ea avformat/nutdec: Don't shrink packet size manually
It is unnecessary because an av_shrink_packet() a few lines below
will set the size; furthermore, it is actually harmful, because
av_shrink_packet() does nothing in case the size already matches,
so that the packet's padding is not correctly zeroed.

currently in AVStreamInternal; or rather: Put AVStream at the
beginning of a new structure called FFStream (which encompasses
more than just the internal fields and is a proper context in its own
right, hence the name) and remove AVStreamInternal altogether.

Andreas Rheinhardt fed0282508 avformat: Avoid allocation for AVFormatInternal
Do this by allocating AVFormatContext together with the data that is
currently in AVFormatInternal; or rather: Put AVFormatContext at the
beginning of a new structure called FFFormatContext (which encompasses
more than just the internal fields and is a proper context in its own
right, hence the name) and remove AVFormatInternal altogether.

The biggest simplifications occured in avformat_alloc_context(), where
one can now simply call avformat_free_context() in case of errors.

Andreas Rheinhardt e318438f2f avformat: Make AVChapter.id an int64_t on next major bump
64 bits are needed in order to retain the uid values of Matroska
chapters; the type is kept signed because the semantics of NUT chapters
depend upon whether the id is > 0 or < 0.

Anton Khirnov cea7c19cda lavf: move AVStream.*index_entries* to AVStreamInternal
Those are private fields, no reason to have them exposed in a public
header. Since there are some (semi-)public fields located after these,
even though this section is supposed to be private, keep some dummy
padding there until the next major bump to preserve ABI compatibility.

Andreas Rheinhardt c1e439d7e9 avformat: Forward errors where possible
It is not uncommon to find code where the caller thinks to know better
what the return value should be than the callee. E.g. something like
"if (av_new_packet(pkt, size) < 0) return AVERROR(ENOMEM);". This commit
changes several instances of this to instead forward the actual error.

Anton Khirnov 83548fe894 lavf: fix usage of AVIOContext.seekable
It is supposed to be a flag. The only currently defined value is
AVIO_SEEKABLE_NORMAL, but other ones may be added in the future.
However all the current lavf code treats this field as a bool (mainly
for historical reasons).
Change all those cases to properly check for AVIO_SEEKABLE_NORMAL.

Anton Khirnov 9200514ad8 lavf: replace AVStream.codec with AVStream.codecpar
Currently, AVStream contains an embedded AVCodecContext instance, which
is used by demuxers to export stream parameters to the caller and by
muxers to receive stream parameters from the caller. It is also used
internally as the codec context that is passed to parsers.

In addition, it is also widely used by the callers as the decoding (when
demuxer) or encoding (when muxing) context, though this has been
officially discouraged since Libav 11.

There are multiple important problems with this approach:
    - the fields in AVCodecContext are in general one of
        * stream parameters
        * codec options
        * codec state
      However, it's not clear which ones are which. It is consequently
      unclear which fields are a demuxer allowed to set or a muxer allowed to
      read. This leads to erratic behaviour depending on whether decoding or
      encoding is being performed or not (and whether it uses the AVStream
      embedded codec context).
    - various synchronization issues arising from the fact that the same
      context is used by several different APIs (muxers/demuxers,
      parsers, bitstream filters and encoders/decoders) simultaneously, with
      there being no clear rules for who can modify what and the different
      processes being typically delayed with respect to each other.
    - avformat_find_stream_info() making it necessary to support opening
      and closing a single codec context multiple times, thus
      complicating the semantics of freeing various allocated objects in the
      codec context.

Those problems are resolved by replacing the AVStream embedded codec
context with a newly added AVCodecParameters instance, which stores only
the stream parameters exported by the demuxers or read by the muxers.

Andreas Cadhalpun ce10f572c1 nutdec: reject negative value_len in read_sm_data
If it is negative, it can cause the byte position to move backwards in
avio_skip, which in turn makes sm_size negative and thus size larger
than the size of the packet buffer, causing invalid writes in avio_read.

Also fix potential overflow of avio_tell(bc) + value_len.

Ganesh Ajjanagadde 7c8fcbbde3 avutil/tree: add additional const qualifier to the comparator
libc's qsort comparator has a const qualifier on both arguments. This
adds a missing const qualifier to exactly match the comparator API.

Existing usages of av_tree_find, av_tree_insert are appropriately
modified: type signature changes of the comparators, and removal of
unnecessary void * casts of function pointers.
