libavcodec/qsvenc: Enable fixed QP configure in qsv CQP runtime

Enable dynamic QP configuration in runtime on qsv encoder. Through
AVFrame->metadata, we can set key "qsv_config_qp" to change QP
configuration when we encode video in CQP mode.
