Merge remote-tracking branch 'qatar/master'

* qatar/master:
  avconv: flush filtered frames before reconfiguring filters
  mov: stsd entries must be at least 16 byte
  mov: detect EOF in mov_read_dref()
  file: return proper error on seek failures
